Job description

Description
As an Affiliated Practice Dental Hygienist, you will provide preventative and restorative oral hygiene care and education to patients of all ages.
Essential Functions:
  • Provides clinical oral hygiene services including dental prophylaxis, scaling, root planning, sealant application, polish teeth and fluoride application, and administer local anesthesia when necessary.
  • Obtains baseline oral hygiene assessment data, based upon accepted principles of oral hygiene and periodontal care.
  • Assists in the oral health management of all clinic patients.
  • Examines patient's surrounding tissues and teeth to determine extent of abnormal condition requiring treatment and determines dental hygiene treatment plan.
  • Records patient-hygienist transactions in the patient's dental record to assure the dental record accurately and completely reflect the nature of the contact, the condition of the patient, and the care or treatment provided.
  • Participates in the Quality Improvement program of the dental department.
  • Assists in the maintenance of all dental areas in compliance with department directive and policies as well as SLFHC policies and procedures relative to infection control, exposure control and safety issues.
  • Ensures the proper disposal of all contaminated or potentially contaminated materials in accordance with department directive and center policy, as well as state and federal regulations. Assure compliance with the clinic's exposure control plan.
  • Washes, sterilizes, and prepares all instruments and work areas according to the Infection Control Protocols
  • Maintains regular and predictable attendance.
  • Perform other duties as assigned.
Qualifications:
Minimum Qualifications:
  • Graduate of an accredited school of dental hygiene.
  • Must be actively engaged in dental hygiene practice for at least 500 hours in each of the 2 years immediately preceding the Affiliated Practice Relationship.
  • Have held an active dental hygiene license for:
    • At least 5 years OR,
    • At least 3 years and hold a bachelor's degree in dental Hygiene.
  • Must complete the required continuing education hours within the first two (2) weeks of employment in accordance with Arizona law:
    • 4 hours in medical emergencies,
    • 8 hours in two of the following: pediatric, special health care needs, preventive dentistry, or public health community-based dentistry.
CERTIFICATES, LICENSES, REGISTRATIONS
  • Current Arizona Dental Hygienist license.
  • Current Arizona license to practice oral hygiene.
  • Radiology/X-Ray Certificate.
  • Eligibility to apply for the Affiliated Practice Dental Hygienist
Preferred Qualifications:
  • Registered Dental Hygienist certificate.
