As a member of our dedicated hospice team, you will engage with families and colleagues, ensuring the highest standards of care and support. Daily interactions will occur through virtual platforms, complementing the therapeutic processes we implement.

Key Responsibilities

As an LCSW, your primary responsibilities will revolve around making assessments and providing tailored support for children, primarily those with disabilities or developmental delays. Specific duties include:

  • Conducting thorough evaluations and assessments of children in early intervention programs.
  • Actively participating in the development and implementation of Individualized Family Service Plans (IFSP), ensuring that family priorities and concerns are central to the process.
  • Monitoring outcomes and progress, adjusting strategies in collaboration with interdisciplinary team members.
  • Providing families with essential information, strategies, and support to enhance their child’s development, utilizing evidence-based practices.
  • Implementing a coaching model within natural environments to promote skill development in children.
  • Building strong rapport with children, families, and team members to foster a nurturing and effective learning atmosphere.
  • Complying with all applicable federal and state regulations to ensure the health and safety of clients and staff.
Patient Population / Client Focus

Your work will focus primarily on children from birth to three years of age, guiding families through the early and critical stages of development with a keen focus on physical, cognitive, social/emotional, and communication skills. The role requires you to understand the unique experiences and needs of children with developmental challenges, ensuring that interventions are tailored to each individual’s circumstances.

Qualifications

To thrive in this role, candidates must meet the following qualifications:

  • A Master’s Degree or Doctorate in Social Work from an accredited institution.
  • Current licensure by the Department of Public Health as an LCSW.
