Director of Case Management
Location: Nashville, Tennessee
Compensation: $109,762 - $135,000 + 17.5% bonus
Sign-On Bonus: $10,000
Relocation: Case by Case
She Recruits is seeking a highly experienced Director of Case Management for a large, fast-paced Level I Trauma hospital. This leader will oversee all case management operations, ensuring coordinated, efficient patient care across the entire facility. The ideal candidate is a seasoned acute-care case management expert with strong leadership capability, data-driven performance management skills, and a demonstrated ability to drive measurable improvements in throughput, length of stay, and quality outcomes.
Role Overview
The Director of Case Management is responsible for directing all departmental functions, ensuring compliance with regulatory guidelines, organizational policies, and industry best practices. This role requires a highly polished, executive-level leader with strong communication and presentation skills who can influence, motivate, and align interdisciplinary teams around shared goals. The director will manage daily operations, oversee the discharge planning process, and drive system-wide performance improvement while ensuring seamless coordination across all hospital departments.
Key Responsibilities
• Lead and manage case management operations across the hospital, including care coordination, discharge planning, and resource utilization
• Ensure departmental compliance with regulatory standards, organizational policies, and external audit requirements
• Drive performance improvement initiatives to reduce length of stay, improve throughput, and enhance patient outcomes
• Engage clinical and administrative stakeholders to align goals, strengthen communication, and ensure optimal resource usage
• Evaluate, coach, and develop staff to create a high-performing, accountable team
• Apply strong analytical skills to monitor data, identify performance gaps, and implement sustainable solutions
• Serve as a subject matter expert, providing leadership, guidance, and strategic recommendations
• Manage multiple priorities with minimal supervision and maintain professionalism in high-pressure or sensitive situations
Required Qualifications
• Bachelor's degree required
• Master's degree in Nursing, Health Administration, or Business Administration preferred
• RN, LCSW, or LMSW required
•• 3+ years of recent acute-care hospital case management experience (within the last year)
• 2+ years of case management leadership experience
• Proven ability to reduce length of stay and manage capacity
• Strong process improvement and data-analysis skill set
• Demonstrated success managing large, complex teams in a high-volume hospital
