The Manager of Case Management is responsible for supporting the Director of Case Management in managing the Case Management operations. This includes providing leadership for planning, management, implementation, integration and coordination of Case Management in support of facility goals dedicated to excellence, effective leadership, and financial stability. Supporting the delivery of the high quality, cost effective patient care through activities based on facility strategic goals and objectives.

Job Summary and Qualifications

ESSENTIAL FUNCTIONS:

  • Under the guidance of the Director, the Manager is to assist in the planning and coordination of Case Management activities, operational efficiency and effectiveness as well as major impacts on all other hospital areas.
  • Assist the Director in providing direction to the staff of the Case Management team and assists in reviewing and evaluating performance of the department staff. Involved in counseling and appropriate disciplinary actions of staff members as requested.
  • Assist department staff in meeting and adhering to all applicable federal, state, JCAHO, and local regulatory agency requirements and assist in the success of the department and facilities in any regulatory survey.
  • Makes decisions in absence of the director to maintain acceptable operations based on strategic goals and policies. Displays ingenuity and foresight in determining the most appropriate solution in the absence of established guidelines.
  • Confers with other department leaders when necessary to resolve procedural difficulties, clarifying department responsibilities, objectives and resolving identified problems.
  • Adheres to hospital/departmental attendance policy, work hours and maintenance of personal appearance as stated in facility policy.
  • Makes suggestions on development of new system processes as needed to support new regulations, software installation, changing environment, and needs of team.
  • Works with the Case Management Director to monitor staff caseload, reorganizing department as needed to support patient population on each unit.
  • Approves staffing schedule ensuring appropriate coverage to support patient case management needs.
  • Monitors daily reports of observation patients. Understands financial implications of Length of Stay requirements for organization and serves to improve efficiency while ensuring safe, appropriate discharge planning throughout the organization.
  • Collaboratively works with the Case Management team and effectively engages employees, maintains positive teamwork relationships, and strives to improve employee engagement.
  • Develops, coordinates and monitors strategies for obtaining, organizing and allocating resources required for care transitions.
  • Assists team members in problem-solving complex patient situations.
  • Establishes collaborative practice with nursing personnel, physicians, and health care professionals regarding the plan of care and identifies variance in plan implementation.
  • Identifies necessary resources or eliminates unnecessary resources needed to achieve quality patient outcomes as well as advocates for patients facing barriers to resources. This includes identifying alternative options if needed resources are not accessible.
  • Manages referrals for post-acute services based on Federal and State regulations, client eligibility for service, the patient’s home care needs and the ability of the agency to safely provide the needed services.
  • Collaborates with healthcare team, develops treatment goals and plans interventions that assist the patient and family to resolve social and psychosocial concerns that may arise as a result of illness, engagement in high risk social behaviors, limited finances, and lack of family support and/or physical disabilities.
  • Collaborates with post-acute discharge providers in the development of transition plans. Obtains physician involvement and agreement with plan. Advocates for patient choice.
  • Provides leadership for multidisciplinary team efforts during interdisciplinary rounds (IDR’s).
  • Works collaboratively with peers to achieve department goals and daily work as evidenced by appropriate and timely communication which is respectful and clear. Sensitive to workload of team, shares responsibilities, fills in and offers help.
  • Serves as a preceptor for new hires, facilitates education and growth within the staff, and actively participates in coaching and evaluation of assigned team members.
  • Actively participates in conferences, committees and task forces as directed.
  • Demonstrates understanding of and adherence to HCA’s Code of Conduct.
  • Demonstrates behaviors that show understanding of HCA Mission and Values and CJW’s Cultural Beliefs.
  • Accepts other assignments and duties as assigned.

What qualifications you will need:

  • BSN or on track for obtaining BSN within 2 years of hire date OR an MSW
  • If RN: Current VA RN license required or appropriate compact licensure.
  • 3-5 years Case Management experience preferred
  • ACM or CCM Certification preferred
