Director of Case Management Opportunity:
  • Full-time, permanent opportunity
  • Schedule: Days; Mon-Fri
  • Oversees case management operations, including care coordination, discharge planning, and utilization review
  • Leads and develops case management staff to support quality outcomes, regulatory compliance, and patient satisfaction
  • Collaborates with physicians, nursing leadership, and interdisciplinary teams to optimize patient throughput and resource utilization
Director of Case Management Qualifications:
  • The Director of Case Management must have an LCSW or RN license
  • Must have 3 or more years of hospital based case management experience
  • Have or be willing to obtain CCM or ACM
  • Bachelor's Degree or higher required
