Description:

The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y.


Jordan Health is now seeking a RN Care Manager-OB/GYN/PEDS who, under the guidance of the OB/Peds Practice Manager, is primarily responsible for providing care organization, education, resource navigation, and clinical support for pediatric and OB/GYN patients to reduce health disparities, support population health goals and the overall improvement of clinical outcomes in patient populations most at risk. This position includes the ability to provide care via telehealth and conduct home visits as appropriate.

Requirements:

The RN Care Manager-OB/GYN/PEDS will ensure that:

· Patients receive consistent, competent, high-quality care

· The organization achieves targeted health outcomes

· Regulatory and financial goals are met

· Access and engagement are optimized


Education And Experience Required:

· Active, unrestricted RN license in NYS.

· Minimum of 2 years of nursing experience, including at least 1 year in pediatrics, OB/GYN, or community/public health.

· Strong communication and organizational skills.

· Proficiency with EHR and data documentation.

· BLS through the American Heart Association

· NYS Infection Control certification.


Special Skills, Knowledge Preferred:

· Experience in care coordination in an FQHC or similar safety-net setting.

· Bilingual in English and Spanish (or other prevalent language).

· Certification in Care management (e.g., CCM ,RN-BC), maternal-child or pediatric nursing.

· Familiarity with HRSA metrics and Medicaid Manage Care.
