Job description

Southern Tier AIDS Program, Inc. is looking for a Ryan White Non-Medical Care Manager to join our Binghamton team! This position providesconfidential non-medical case management services to persons living with diagnosed Human Immunodeficiency Virus (HIV). Improves participant access to and retention in core medical and support services. Provides coordination, guidance, and assistance in accessing medical, social, community, legal, financial, employment, vocational, and/or other needed services.The Ryan White Non-Medical Care Manager reports directly to the Director of Programs, Intensive Services.

Responsibilities

The primary responsibilities and duties of a Ryan White Non-Medical Care Managerinclude provision of care management services to persons living with HIV, as well as their families, and collaterals who receive services from STAP. This includes but is not limited to:

  • engaging clients in care management services, including: orientation, initial intake, assessment, re-assessments, service plan development and implementation, care coordination, and follow-up activities
  • referrals to RW Non-Medical Case management, Health Education/Risk Reduction, Medical
    Transportation, Psychosocial Supportive Services, Housing-Short Term Rent and Utility Assistance, Emergency Financial Assistance, Food Bank/Home Delivered Meals
  • assisting with obtaining needed linkages and referrals including: medical, legal, housing, social, financial, transportation, food security and other requested services
  • serving as client advocate as necessary
  • arranging care conferences with team members as necessary (minimally every 6 months)
  • completing sufficient, accurate, and timely documentation of all interventions, activities and
    communications during the above listed activities and all ongoing contacts with client
  • entering and verifying the accuracy of data for all client interactions, as well as referral management, and crisis planning
  • gathering client feedback about services and assessing client satisfaction
  • filing paperwork into client charts so that every chart is the same and easy to access

This position requires a thorough understanding of the admission/eligibility process and criteria; a basic knowledge of the Ryan White HIV/AIDS program is essential.
