Pay: $40,000–$50,000 per year

Help Clients Turn Recovery Into Real-Life Progress

Milestone Treatment Center is seeking a compassionate, resourceful, and highly organized Case Manager to support individuals receiving treatment for substance use and mental health conditions.

Treatment is only one part of recovery. Clients may also be navigating housing, employment, insurance, family responsibilities, transportation, healthcare, benefits, and other real-world challenges. That's where you come in.

As a Case Manager, you'll help clients identify what they need, connect them with the right resources, and remove barriers that could interfere with their treatment and long-term stability.

What You'll Do
  • Build strong, professional relationships with clients and serve as a consistent source of support, guidance, and advocacy.

  • Assess each client's individual needs, challenges, strengths, and available resources.

  • Develop, implement, and monitor individualized case management plans.

  • Connect clients with community resources related to housing, employment, healthcare, behavioral health, transportation, benefits, and other support services.

  • Help clients navigate complex systems and processes, including housing programs, government assistance, insurance, and community services.

  • Assist with verification of benefits and related coordination when required.

  • Provide support and appropriate intervention during challenging or crisis situations.

  • Facilitate groups or educational sessions as needed.

  • Advocate for clients and help eliminate barriers that could interfere with treatment, recovery, and successful transition back into the community.

  • Coordinate with outside providers, agencies, families, and community organizations when appropriate.

  • Collaborate closely with therapists, medical providers, counselors, and other members of our multidisciplinary treatment team.

  • Maintain timely and accurate case notes, documentation, referrals, and client records.

  • Track client progress and follow through on outstanding needs and resources.

  • Maintain appropriate professional boundaries and client confidentiality.

  • Follow applicable HIPAA, 42 CFR Part 2, regulatory, ethical, and Milestone requirements.

Qualifications
  • Experience in case management, social work, behavioral health, substance use treatment, mental health, crisis intervention, or human services is strongly preferred.

  • Active or pending LCADC or QMHP-A credential preferred.

  • Experience connecting individuals with community resources and support services is highly valued.

  • Strong interpersonal and relationship-building skills.

  • Excellent verbal and written communication skills.

  • Ability to advocate effectively while maintaining appropriate professional boundaries.

  • Strong organizational skills and attention to detail.

  • Ability to manage multiple clients, priorities, and follow-up activities.

  • Comfortable working independently while contributing to a multidisciplinary team.

  • Ability to remain calm and solution-focused when clients encounter difficult situations.

  • Proficiency with Google Workspace and comfort learning electronic documentation systems.
