Manager, Case Management

Position Summary

We are seeking an experienced Manager of Case Management to lead and coordinate care management, utilization review, discharge planning, and clinical quality initiatives within a leading acute care hospital system.

The Manager is responsible for overseeing the systems, processes, and daily operations that support effective utilization management and patient care coordination. Working closely with the Director, this leader will help drive departmental performance, optimize resource utilization, improve patient outcomes, and ensure compliance with regulatory and payer requirements.

Additional responsibilities include participating in the collection, analysis, and reporting of financial, utilization, quality, and performance improvement data while fostering collaboration among physicians, nursing staff, social services, and other healthcare professionals.

Qualifications

Education

  • Bachelor's Degree in Nursing (BSN) or Social Work (BSW) required
  • Master's Degree preferred

Licensure & Certifications

  • Current Texas RN License OR Texas LMSW/LCSW License required
  • Case Management Certification required

Experience

  • Minimum of five (5) years of experience in:
  • Case Management
  • Utilization Management
  • Discharge Planning
  • Cost and Quality Management Programs
  • Minimum of three (3) years of hospital-based nursing or social work experience
  • Minimum of three (3) years of leadership or management experience

Knowledge, Skills & Abilities

  • Strong understanding of utilization management, case management, managed care, and the continuum of care including inpatient, outpatient, and home health services
  • Knowledge of payer requirements and evidence-based clinical care practices
  • Experience with performance improvement and quality management initiatives
  • Proven ability to develop and execute operational plans with measurable outcomes
  • Demonstrated success building collaborative relationships with physicians and interdisciplinary teams
  • Strong analytical, organizational, and problem-solving abilities
  • Excellent verbal and written communication skills
  • Self-motivated, proactive, and results-oriented leadership style
