Position Overview

We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team at Redesign Counseling Hub in Medford, Oregon. Work Environment

Regular virtual meetings and coordinated discussions with healthcare colleagues will be essential to maintain a cohesive treatment approach.

Key Responsibilities
  • Collaborate with a multidisciplinary healthcare team to develop comprehensive treatment and discharge plans that align with the psychosocial assessment and established social work interventions.
  • Engage with community resource organizations, representing Redesign Counseling Hub in meetings and committees to advocate for the social needs of our patients.
  • Provide counseling and crisis intervention services to hospice patients, effectively assessing and addressing emotional and behavioral challenges.
  • Maintain open communication with physicians regarding patient progress, including updates on needs, concerns, and any emerging problems.
  • Ensure accurate documentation and record-keeping, including clinical notes and progress reports, in strict adherence to departmental policies.
Patient Population / Client Focus

This role focuses on patients receiving hospice care, often dealing with complex emotional and psychological issues related to end-of-life concerns. You will support individuals and their families during a challenging time, providing the tools and guidance necessary to improve their quality of life and emotional well-being.

Qualifications
  • A Master’s degree in social work or a related field is required, along with valid licensure as an LCSW.
  • A minimum of three years of experience working in an outpatient or similar clinical setting, preferably within hospice or palliative care.
  • Demonstrated ability to foster and maintain professional relationships with fellow therapists, medical professionals, and patients.
  • Strong understanding of relevant theories, assessment tools, and evidence-based therapeutic modalities.
  • Exceptional communication skills, both verbal and written, ensuring clarity when discussing patient needs and treatment plans.
Preferred Experience
  • Previous experience in end-of-life care, hospice settings, or working with bereaved families is highly desirable.
  • Knowledge of local community resources and support services that can benefit patients and families.
  • Familiarity with electronic health record systems (EHR) to streamline documentation and patient management.
Compensation and Benefits
