Position Overview

Redesign Counseling Hub is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) for a full-time remote position. Key Responsibilities

As an LCSW at Redesign Counseling Hub, your responsibilities will include:

• Collaborating closely with an interdisciplinary healthcare team to develop and implement personalized treatment plans that address the specific psychosocial needs of each patient.
• Conducting thorough psychosocial assessments and creating discharge plans that inform ongoing treatment and resource allocation.
• Leading crisis intervention efforts, utilizing your experience to assess emotional and behavioral concerns thoroughly and constructively.
• Acting as a liaison between patients and their healthcare providers, effectively communicating patient progress, emerging concerns, and necessary adjustments to treatment plans.
• Maintaining meticulous clinical records, including accurate clinical notes and progress updates in accordance with departmental policies and best practices.

Patient Population / Client Focus

Your work will serve a diverse patient population, involving children, adolescents, adults, and families who require mental health support. You will address a wide spectrum of issues, from anxiety and depression to trauma and relationship challenges. This role will allow you to have a meaningful impact by assisting patients in overcoming barriers to their mental health.

Qualifications

Candidates for this position must possess:

• A Master’s degree in Social Work (MSW) and hold a current and valid Licensed Clinical Social Worker (LCSW) credential.
• At least three years of experience working in an outpatient mental health setting, demonstrating a commitment to high-quality patient care.

Preferred Experience

Compensation and Benefits
