Job description

Client Care Coordinator (Minimum 2 Years Experience Required) - Home Instead
Home Instead is seeking an Experienced Client Care Coordinator who is highly energetic and organized to join our team. Applicants must have an active CNA license and a minimum of two (2) years of Client Care Coordination experience. If you're passionate about improving the lives of aging adults and thrive in a fast-paced, team-oriented environment, we'd love to hear from you.
Please Note: This position requires a minimum of two (2) years of Client Care Coordination experience. Applicants who do not meet this minimum requirement will not be considered.
Position Overview
As a Client Care Coordinator, you will oversee quality assurance and interact directly with clients and their families. You'll be responsible for managing multiple demands, ensuring client satisfaction, and adapting to clients' evolving needs. This is a full-time hybrid position combining office and field work (not work from home) and requires strong computer skills and the ability to learn new software platforms.
Primary Responsibilities
  • Conduct quality assurance visits with clients and families.
  • Address and resolve client concerns to ensure satisfaction.
  • Introduce caregivers to clients and ensure proper training and support.
  • Perform supervisory visits with caregivers to assess performance and provide coaching and training as needed.
  • Help grow the business by identifying opportunities to modify service plans to best support clients' ongoing needs.
  • Hybrid role between field work and the office (not work from home).
Minimum Requirements to Apply
  • Active CNA license (required).
  • Minimum of two (2) years of Client Care Coordination experience (required). Applicants without this experience will not be considered.
  • Current driver's license and proof of auto insurance.
  • Ability to work in a fast-paced environment and effectively manage competing priorities.
  • Strong communication skills with a service and solution-oriented mindset.
  • Proficient computer skills and the ability to quickly learn new software platforms.
  • Available to work a Monday-Friday schedule during the first 30 days of training, followed by a rotating weekend schedule with the second Client Care Coordinator.
  • Full-time salaried position.
  • A current resume is required to be considered for this position.

  • Demonstrated ability to think critically, prioritize competing demands, and make sound decisions independently.
  • Strong conflict resolution and problem-solving skills, with the ability to navigate difficult conversations professionally.
  • Experience supervising, coaching, mentoring, or developing caregivers or healthcare staff.
  • Proficient using multiple software platforms and technology in a fast-paced environment.
  • Demonstrated initiative and a strong sense of ownership, with the ability to anticipate needs, identify solutions, and follow through with minimal supervision.
  • Self-motivated, adaptable, and able to thrive in a collaborative team environment.
