Sentara Wellness Center is seeking a committed and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team in a full-time remote position. Key Responsibilities
  • Conduct in-depth psychosocial assessments to identify clientsโ€™ mental health concerns, strengths, and challenges, ensuring a comprehensive understanding of their needs.
  • Facilitate individual, family, and group therapy sessions, utilizing evidence-based approaches such as cognitive-behavioral therapy (CBT) and motivational interviewing, tailored specifically to each clientโ€™s cultural and individual background.
  • Formulate detailed treatment plans that set measurable goals and include interventions designed to support client progress, with regular monitoring and adjustment as necessary.
  • Work collaboratively with multidisciplinary teams, including psychiatrists, nurses, and other professionals, to develop and implement cohesive treatment strategies that address the whole-person care approach.
  • Provide immediate crisis intervention services, conduct risk assessments, and create safety plans for clients in emergency situations to ensure their safety and well-being.
  • Act as an advocate for clients, assisting them in obtaining community resources and personalized case management support tailored to their unique situations.
  • Participate actively in community outreach and engagement activities aimed at increasing awareness and understanding of mental health issues in the Syracuse area.
