Key Responsibilities
  • Conduct comprehensive psychosocial assessments for seniors, capturing critical emotional, mental, and social factors affecting their well-being.
  • Collaborate with clients and their families to establish individualized care plans that reflect their goals, preferences, and readiness for change.
  • Provide counseling and emotional support that addresses specific life transitions, such as loss, disability, or relocation to assisted living facilities.
  • Advocate for client rights, ensuring they have access to vital community resources, including transportation, support groups, and recreational programs.
  • Work collaboratively with an interdisciplinary team to enhance the quality of care, involving regular care meetings to discuss client progress and needs.
  • Perform regular virtual visits to monitor changes in clients' mental health and ensure their living conditions meet safety standards.
  • Facilitate discussions among family members to address conflicts and improve care strategies for client wellness.
  • Educate clients and their families about available community resources to enhance their engagement and social interactions.
  • Maintain meticulous records of client interactions, progress notes, and care plans in compliance with ethical standards and best practices.
  • Stay updated with current social work practices, actively participating in webinars and training for continuous professional development.
Patient Population / Client Focus

Your primary focus will be on senior clients, typically aged 65 and above, who may face mental health issues, emotional upheaval, or challenges related to aging. These clients often require assistance in adjusting to lifestyle changes, managing chronic illnesses, and maintaining their independence. Your empathetic approach will be crucial in establishing trust and rapport, facilitating open discussions that promote their mental and emotional health.

Qualifications
  • Possession of a Masterโ€™s degree in Social Work from an accredited program.
  • Valid and current licensure as a Licensed Clinical Social Worker (LCSW).
  • At least two years of experience in social work, preferably within a community health or geriatric setting, demonstrating a commitment to serving older adults.
  • In-depth knowledge of social work principles, particularly related to aging and mental health issues, as well as familiarity with community resources available for seniors.
  • Exceptional assessment and therapeutic skills to effectively address client needs.
