Key Responsibilities
• Provide support to the systems that deliver resources and services to patients.
• Maintain appropriate documentation in State Database to reflect services rendered to clients under Ryan White Part B Funding.
• Provide timely payment of medical bills, transportation costs, food-based services, emergency financial spending, and housing-related expenses as appropriate, ensuring all payments are documented accurately.
• Assist with housing-related services for clients, including coordinating housing payments, supporting housing stability efforts, and helping facilitate housing inspections when required.
• Assist clients with obtaining timely and coordinated access to medically appropriate levels of health and support services and continuity of care.
QUALIFICATIONS
• Must have a Bachelor's Degree in case management or related field, or equivalent experience.
• S/he should have a basic knowledge of medical case management principles.
• Experience working in a nonprofit organization or a strong desire to work in service driven environment.

• Flexibility, as our services are available to a large geographic area in the state of Kentucky, which may require some travel
• Valid Driver's License with at least three (3) years driving experience and less than two (2) moving violations in the past three (3) years.
• Must be able to pass a drug screen and background test.
