Hinds Lifecare, one of the most well-known and well-respected nonprofit agencies in California's Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a LVN Intake Coordinator to join our team.
The LVN Intake Coordinator is responsible for taking all hospice information calls regarding the intake of new referrals to facilitate an evaluation for hospice service. The LVN manages all patient intake functions throughout the entire intake process and provides Hospice evaluations as directed.

Qualifications include but are not limited to:
  • Completion of a Licensed Vocational Nursing Education Program.
  • Demonstrated ability to work with health team personnel.
  • Minimum 1 year experience in acute care facility or community health agency, (prefer one-year hospice experience).
  • Current LVN license, CPR, recent physical and TB, Tdap, MMR, Varicella, HepB immunizations.
  • Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs).
  • Ability to learn and assimilate the NetSmart Computer Program/System and navigate through multiple technical interface portals to enter and retrieve information related to patient care.
  • Available Tuesday through Saturday, 9am to 5pm.
