Requirements & Responsibilities: 

  • Graduate of an accredited nursing program (Associates, Bachelors, or Masters) 
  • Current Registered Nurse (RN) license in the state of practice 
  • 2+ years of hospice or home health clinical experience required 
  • 2+ years of supervisory or management experience required 
  • Oversee coordination of patient care and assign appropriate staff to each case 
  • Support clinicians with real-time guidance, mentoring, and clinical problem-solving 
  • Conduct case reviews, home visits, and clinical audits to ensure high-quality care 
  • Manage hiring, training, supervision, and evaluation of clinical staff 
  • Assist in ongoing staff education, orientation, and performance improvement efforts 
  • Must have reliable transportation, valid driver’s license, and proof of auto insurance
