Community Care is hiring a RN Case Manager to join our Family Care Program team. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, being part of a mission driven organization and who shares our mission to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This is a full-time opportunity working out of our Waukesha location.
*****$10,000 Sign On Bonus******
RN Case Manager Job Responsibilities:
  • Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed.
  • Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.

RN Case Manager Job Requirements:
  • Education: Associates degree in Nursing Required, Bachelors Degree preferred.
  • Licensure: Wisconsin Licensed Registered Nurse
  • Licensure: Eligibility for long-term care functional screener certification is required
  • Licensure: WI Drivers License
  • Experience: Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity Required
    Preferred 2 years of work in community nursing

RN Case Manager Job Schedule:
Monday- Friday 8:00 AM-4:30 PM.
