This full-time position is pivotal in providing essential mental health services to individuals and families dealing with challenges related to hospice care. • Conduct thorough assessments to evaluate clients' emotional, psychological, and social needs, ensuring a holistic understanding of their circumstances.
• Provide compassionate counseling to individuals and families, helping them navigate the complexities of end-of-life care.
• Utilize evidence-based practices to offer individual and group therapy sessions, addressing grief, loss, and transition phases associated with hospice care.
• Develop personalized care plans in collaboration with clients, setting clear goals and objectives that align with their needs.
• Advocate for clients' rights and facilitate access to necessary resources, ensuring that all emotional and practical aspects of care are addressed.
• Engage in program development and evaluation, providing feedback to enhance service delivery and optimizing client outcomes.
• Work closely with health care providers, community partners, and volunteers, fostering a collaborative environment to improve service accessibility and effectiveness.
• Participate in ongoing training, workshops, and community outreach to stay informed about best practices in social work and hospice care.

The primary focus of this role is to support individuals and families who are experiencing the end-of-life journey. Your expertise will aid in alleviating emotional distress, facilitating communication, and providing coping strategies during a challenging time. You will also engage with diverse clients, including those from marginalized groups, ensuring equitable access to support services.

• A Master’s degree in Social Work from an accredited institution is required.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory to practice in Nevada.
• Strong understanding of hospice care and its impact on patients and families is essential.
• Exceptional communication, empathy, and active listening skills necessary for effective counseling.
• Proficient in utilizing virtual communication and case management tools to support remote service delivery.

• Experience working in a nonprofit or healthcare setting, particularly in hospice or palliative care environments.
• Knowledge of trauma-informed care practices and community resources relevant to end-of-life services is an asset.
• Previous involvement in advocacy efforts or policy development related to social justice and healthcare access.
