Key Responsibilities
  • Conduct thorough evaluations to assess each patientโ€™s functional abilities and needs, using standardized assessment tools to develop a comprehensive understanding of their challenges.
  • Design and implement individualized treatment plans, establishing clear goals that focus on enhancing activities of daily living (ADLs), mobility, and cognitive function.
  • Utilize evidence-based therapeutic interventions and adaptive techniques, such as assistive technology and sensory integration methods, tailored to the specific needs of each resident.
  • Educate patients and their families about adaptive strategies and home modifications that can facilitate recovery and promote independence.
  • Collaborate with our diverse healthcare team, sharing insights and contributing to integrated care plans that optimize treatment outcomes.
  • Document patient progress meticulously, making necessary adjustments to treatment plans based on evolving patient needs and responses.

Qualifications
