Job description

Lighthouse Guild is the leading not-for-profit healthcare organization dedicated to addressing and preventing vision loss. Our mission is to provide exceptional services that inspire people who are visually impaired to attain their goals. Our vision is a world in which no person is limited by their visual capacity. Our Rehabilitation Department is seeking a part time Social Worker/LMSW (21 hours/week, in-person, flexible days Monday-Friday). Bilingual Spanish/English fluency is required.

Position Summary:

The role provides a full range of social work services for individuals who are blind and visually impaired including case management and concrete services while coordinating with internal and community-based service providers.  Provides short term individual and group psychotherapy focusing on adjustment to vision loss. Bi-lingual Spanish/English fluency required.

Duties/Responsibilities:

          1.  Assesses the needs of assigned clients and devises a plan to assist in meeting these needs.

2.  Makes referrals for adjunct services, completes applications for entitlements, assists with accessing community resources.

3.  Outreaches to social service agencies, housing agencies and other support services that may be needed by clients.

4.  Responds to client phone inquiries and provides information and referral.

5.   Facilitates meetings with NYS Commission for the Blind (NYSCB) vocational rehabilitation counselor and client after assessment and at the  conclusion of services.

6.  Writes assessment and progress reports following NYSCB guidelines and submits to supervisor for review.

7.  Provides intensive individual brief psychotherapy and group work with a primary focus of adjustment to vision loss in accordance with NYSCB guidelines.

8.  Refers to Lighthouse Guild behavioral health or other community-based provider for long term services if indicated.

9.  Meets with supervisor as scheduled for case reviews.

10. Manages a caseload consisting of parents of young children with vision loss, teenagers and adults.

11.  Performs miscellaneous duties as assigned by supervisor.

Required Skills/Abilities

  • Works with clients who require consistent and comprehensive support
  • Remains in a stationary position at a workstation and use a computer approximately 90% of the time
  • Excellent English language writing skills

Education and Experience:

  • Master’s Degree in Social Work – licensed (LMSW)
  • Bi-lingual Spanish/English
  • Experience working with individuals with disabilities preferred
