Key Responsibilities
  • Act as a collaborative partner with primary care providers to identify, treat, and manage mental health and psychosocial issues.
  • Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, and recommend tailored treatment goals to the primary care team.
  • Provide brief, focused interventions for patients seeking mental health services, ensuring timely and effective support.
  • Evaluate patient progress, initiating follow-up assessments to modify treatment plans as necessary, ensuring optimal mental health outcomes.
  • Collaborate with the primary care team to develop and execute relapse prevention plans where indicated, promoting sustained patient well-being.
  • Recognize 'at-risk' patients and develop strategic plans aimed at preventing deterioration of their mental health conditions.
  • Coordinate behavioral health services and provide referrals to specialized mental health facilities when necessary.
  • Accurately document patient progress and maintain detailed clinical records to aid in ongoing treatment decisions.
  • Provide education to patients regarding self-management skills, enabling them to actively participate in their treatment and recovery journeys.
  • Work in accordance with the ethical standards outlined in the Code of Ethical Conduct for Clinical Professionals.
