• Act as a collaborative partner with primary care providers to identify, treat, and manage mental health and psychosocial issues.
• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, and recommend tailored treatment goals to the primary care team.
• Provide brief, focused interventions for patients seeking mental health services, ensuring timely and effective support.
• Evaluate patient progress, initiating follow-up assessments to modify treatment plans as necessary, ensuring optimal mental health outcomes.
• Collaborate with the primary care team to develop and execute relapse prevention plans where indicated, promoting sustained patient well-being.
• Recognize 'at-risk' patients and develop strategic plans aimed at preventing deterioration of their mental health conditions.
• Coordinate behavioral health services and provide referrals to specialized mental health facilities when necessary.
• Accurately document patient progress and maintain detailed clinical records to aid in ongoing treatment decisions.
• Provide education to patients regarding self-management skills, enabling them to actively participate in their treatment and recovery journeys.
• Work in accordance with the ethical standards outlined in the Code of Ethical Conduct for Clinical Professionals.
• Perform additional duties as assigned, contributing to overall clinic effectiveness.

You will engage with a diverse patient population, primarily focusing on individuals seeking mental health support in conjunction with their primary care needs. By utilizing your skill set and compassion, you will foster positive therapeutic relationships that encourage patient autonomy and engagement in their recovery process.

• A minimum of a Master’s Degree in Clinical Counseling.
• A current LPC (Licensed Professional Counselor) License, ensuring adherence to state licensing boards and regulations.
• In-depth knowledge of mental health assessment methods, treatment planning, and interventions based on developmental psychopathology.
• Expertise in psychodiagnostics, psychotherapeutic techniques, and behavioral therapy approaches.
• Strong assessment skills to determine individual patient needs and evaluate care efficacy.
• Capacity to maintain accurate clinical records and generate professional reports.
• Effective communication and cooperative teamwork abilities to liaise with healthcare professionals and community resources.

• Bilingual proficiency in English and Spanish is preferred to serve a broader demographic, enhancing accessibility of mental health services.
• A comprehensive understanding of community resources and their application for patient care.
• Experience in working within an interdisciplinary team setting, bringing unique insights into patient care planning.

Life Solutions Services offers a competitive salary range of USD 87,000 – 105,000 annually, commensurate with experience and qualifications.
