Key Responsibilities
  • Act as a pivotal liaison between clients, families, and outpatient providers, ensuring seamless communication and care coordination.
  • Interact with insurance companies for pre-certification and utilization management to authorize necessary services for clients.
  • Engage in vital discharge and aftercare planning, supporting clients as they transition from intensive therapies back into their communities.
  • Provide therapeutic meal support, helping clients establish a healthier relationship with food, by modeling positive behaviors during shared meal times.
  • Deliver client-centered care that is empathetic, affirming, and tailored to the unique circumstances of each individual, particularly focusing on their intersecting identities.
Patient Population / Client Focus

This role primarily focuses on serving a diverse population, including older adults facing mental health challenges, emotional distress, and interpersonal conflicts. The LCSW will work diligently to understand the unique backgrounds of each client, including cultural, socioeconomic, and health-related factors that influence their mental health journey.

Qualifications
  • A Master's degree in Social Work obtained from an accredited institution is required.
  • Must possess and maintain a valid LCSW license in Pennsylvania.
  • Experience working with older adults or within geriatric services is essential.
  • Strong clinical and diagnostic skills are mandatory for effective client assessment and intervention.
  • Exceptional communication and interpersonal skills to foster strong relationships with clients, families, and colleagues.
