Job description


Position Summary

The DTA Care Worker provides habilitative training, supervision, and personal care to adult members with developmental disabilities for part of the day, helping them acquire, retain, or improve socialization, communication, and adaptive living skills to participate in the community. Services are delivered per each member’s Person-Centered Service Plan (PCSP/ISP), in accordance with DDD DTA service specifications and Qualified Vendor standards.

Essential Duties & Responsibilities

  • Implement individualized goals and outcomes from each member’s PCSP/ISP; provide specialized sensory-motor, cognitive, communicative, social interaction, and behavioral training consistent with DTA service definitions.
  • Facilitate community integration and access to resources (e.g., libraries, parks, shops), promoting meaningful days, valued roles, and social relationships.
  • Provide personal care, feeding, etc. assistance as needed at DTA location.
  • Support therapy carryover when therapy occurs at the DTA site with staff present and learning implementation, or when a caregiver/member representative participates
  • Other duties as assigned by the DTA operation manager/supervisor

Health, Safety, & Risk Management

  • Adhere to agency health/safety policies including medication practices, incident identification/reporting, abuse/neglect prevention, infection control, and emergency procedures; complete Incident Reports and required notifications within 24 hours of occurrence (or immediately for serious incidents) per DDD guidance.
  • Maintain visual supervision and implement enhanced ratios only as authorized in the PCSP/ISP (e.g., 1:1 or 2:1).
  • Follow prohibited practices and safety requirements applicable to provider staff (training topics include CPR/First Aid, emergencies, incidents, medications, abuse/neglect).

Documentation & Communication

  • Accurately document daily attendance, services delivered, member progress toward outcomes, and any incidents or health changes; submit timely, complete records per agency and DDD requirements.
  • Communicate effectively with members, guardians/representatives, support coordinators, clinicians, and agency leadership to ensure coordinated, person-centered services.

Minimum Qualifications

  • 18 years of age or older; professional references; knowledge/skills/experience sufficient to perform DSP duties.
  • Fingerprint Clearance Card and statement regarding criminal history per Arizona rules; ongoing compliance with fingerprint requirements.
  • Current CPR and First Aid certifications from certified instructors (completed before working independently with members).
  • Completion of agency-required orientation and training curriculum covering member-specific needs, health/safety policies, incidents, abuse/neglect, medication procedures, emergency response, and prohibited practices.
  • HCBS certification eligibility (agency/individual) as required by the Office of Licensing, Certification and Regulation (OLCR) for DTA/HCBS service provision.
  • Valid Arizona driver’s license (if transporting members), acceptable driving record, and ability to meet agency transport policies.

Work Schedule

  • Mondays- Fridays (7AM - 5 PM)
