• Conduct thorough assessments to determine the psychosocial needs of patients in a hospice setting, particularly in relation to substance abuse and mental health challenges.
• Develop personalized treatment plans utilizing evidence-based therapies and interventions including Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT).
• Provide ongoing psychotherapy sessions to individuals and families dealing with substance abuse issues, ensuring a safe and supportive therapeutic environment.
• Maintain accurate clinical records in compliance with state regulations and organizational standards.
• Collaborate with a multidisciplinary team including nurses, physicians, and other social workers to ensure holistic care for patients.
• Educate patients and their family members on available mental health and substance abuse resources, empowering them to make informed decisions about their care.
• Stay abreast of the latest research and integrate these findings into practice to enhance treatment outcomes.

As an LCSW at Evolve Social Work Center, you will work primarily with individuals who are in recovery from substance abuse and who are navigating the complexities of hospice care. • A Master’s Degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Virginia.
• Minimum of 1 year clinical experience, preferably in substance abuse therapy.

• Excellent communication, problem-solving, and teamwork abilities.

• Familiarity with hospice care principles and practices.

This position offers a competitive salary range of $80,000 to $100,000 annually, commensurate with experience.
