Through transitional housing, programs and supportive services, we provide families in Washington State with opportunities for life change to overcome trauma, generational poverty and situational homelessness. Job Summary: The Case Manager provides advocacy, support, and resources to families in Vision House's transitional housing program. This role focuses on identifying client strengths, addressing barriers, and developing strategies for long-term success. The Case Manager assists residents in achieving goals related to health, employment, finances, education, and permanent housing while maintaining a trauma-informed, client-driven, and strength-based approach to care.
REPORTS TO: Family Program Site Manager
HOURS: 40 hours per week, hourly, Monday-Friday based on program needs
LOCATION: Renton, WA

As a Case Manager, you will:
  • Provide case management demonstrating a trauma-informed, client-driven, strengths-based, and team approach in philosophy of care.
  • Meet with residents weekly to support their goals in health, employment, finances, education/vocation, and permanent housing.
  • Offer resources and advocacy for families, women, men, and couples experiencing homelessness.
  • Assist in maintaining a healthy, alcohol- and drug-free environment.
  • Assist with screening and interviewing of clients.
  • Assist with crisis intervention and referrals for residents in need.
  • Participate in interventions, including voluntary and involuntary terminations.
  • Update client database tracking system (Apricot) and client case notes in a timely manner (within 1 week).
  • Maintain contact with agencies and with community resources that accept VH referrals for housing and employment.
  • Participate in weekly supervision and staff meetings.
  • Conduct regular apartment inspections to ensure safety and program compliance.
  • Complete housing authority paperwork as needed and provide necessary reports.
  • Help organize and implement community meetings, resident classes and family events.
  • Assume other responsibilities as assigned or required.

Requirements
Case Manager candidates should have the following qualifications:
  • Preferred Experience: B.A. Degree in Social Service field, or A.A. degree combined with exceptional experience.
  • Two years of experience must be direct social service or advocacy work, including assessing client needs and/or determining client benefit eligibility
  • Working knowledge of substance use disorders and recovery, preferred
  • Strong listening and communication skills with the ability to interact effectively in diverse situations. Demonstrated ability to work effectively and flexibly under stressful conditions.
  • Demonstrated ability to be consistent and follow-through on case management tasks.
  • Demonstrated ability to work with people of diverse socio-economic backgrounds, ethnicities, beliefs, sexual orientation and lifestyle.
  • Proven ability to work independently and in a collaborative team environment.
  • Experience in crisis intervention and providing resources to individuals in need.
  • Personal qualities of approachability, empathy, sensitivity and flexibility.
  • Proficiency in Microsoft Word, Access, and Excel. Preferred experience with Apricot software
  • Valid Washington State driver's license and reliable transportation.
  • Must be willing to work a flexible schedule depending on program needs.
  • Ability to work 1-2 evenings a week.
  • Ability to pass background check.
  • Active follower of Jesus Christ and have ability to sign our Statement of Faith.

Physical Demands/Work Requirements:
  • Must be able to lift and move 25 pounds.
