Job Title: CCC Program Manager


As the Program Manager, you will play a key leadership role in guiding and advancing CRC’s Case Management Program. This position offers the opportunity to expand your leadership scope, drive program operations, and make a meaningful impact on the lives of individuals with brain injuries and complex needs. You will oversee program performance, support and develop staff, and ensure the delivery of high-quality, client-centered services.


About Community Rehab Care (CRC):

Our mission is to provide high quality, individualized, and community-based outpatient rehabilitative and support services to people with neurologic, musculoskeletal, or orthopedic injuries and illnesses. We strive to support our clients in leading meaningful lives after their injuries.


Job Duties and Responsibilities:

In partnership with the General Manager, the Program Manager is responsible for the day-to-day leadership, operations, and performance of CRC’s Case Management Program, including services provided under the Massachusetts Statewide Head Injury Program (SHIP). This role combines program oversight, fiscal management, and staff leadership, along with a limited caseload.


Key Responsibilities

Program Oversight & SHIP Management

  • Oversee coordination and compliance of services under the Massachusetts Statewide Head Injury Program (SHIP), which is administered by MassAbility (MBY)
  • Review referrals, documentation, and service plans to ensure quality and alignment with program requirements
  • Collaborate with SHIP Regional Supervisors, SHIP Regional Coordinators, and other MBY state partners on service delivery, budgets, and program updates
  • Develop and manage annual and ongoing budget requests and amendments, monitor performance against fiscal targets. Collaborate with GM on final budget requests.
  • Assist GM in annual internal and external company fiscal audits.


Fiscal & Operational Management

  • Monitor staff productivity, billing, and service utilization to ensure program effectiveness
  • Review and approve billing for accuracy and compliance with contractual expectations
  • Identify opportunities to improve efficiency, quality, and program performance


Leadership & Staff Development

  • Provide direct supervision to the Case Management Supervisor and indirect oversight of Case Managers
  • Support staff in complex decision-making and client situations
  • Lead recruitment, hiring, onboarding, and retention efforts in collaboration with GM and CM Supervisor
  • Conduct performance evaluations for direct reports and oversee team performance management processes


Collaboration & Representation

  • Represent CRC in the community and maintain relationships with key stakeholders and referral sources
  • Collaborate with internal leadership, including the General Manager, Director of Rehabilitation, and HR, to support organizational goals


Direct Service (Limited Caseload)

  • Maintain a small caseload of clients, providing comprehensive case management services, including:
  • Assessment and service planning
  • Coordination of medical, behavioral health, and community-based supports
  • Assistance with benefits, housing, and resource navigation
  • Crisis intervention and mandated reporting as required


Professional Development & Program Growth

  • Stay current with best practices, trends, and resources in brain injury and community-based case management
  • Identify and implement opportunities for program development and staff education


Leadership Structure

  • Program Manager (this role – Reports to General Manager)
  • Case Management Supervisor (reports to Program Manager)
  • Case Managers


Job Relationships:

  • Reports to and works in partnership with the General Manager. Directly supervises the Case Management Supervisor and provides indirect oversight to Case Management staff.


Qualifications:

  • Bachelor’s degree required; Master’s degree preferred in a related field (Social Work,
  • Rehabilitation Counseling, Psychology, Occupational Therapy, Nursing, Education, or similar)
  • Certified Case Manager (CCM) and/or Certified Brain Injury Specialist (CBIS) preferred
  • Minimum of 5 years of management or supervisory experience
  • Minimum of 3 years of experience in a community-based healthcare or human services setting
  • Experience working with individuals with brain injuries, disabilities, mental health conditions, and/or substance use disorders strongly preferred
  • Knowledge of Massachusetts state agencies and service systems preferred

Work Environment
