• Psychosocial Assessment: Conduct thorough psychosocial assessments in a timely manner, utilizing theoretical knowledge of human behavior and psychosocial factors to inform treatment plans and interventions. Address ethical considerations related to patient care, including risk issues.
• Clinical Intervention: Provide supportive counseling to patients and their families, facilitating the navigation of challenging decisions such as hospice care and placement options. Assist in communicating patient needs and concerns with healthcare providers, and collaborate with case managers on discharge planning.
• Caseload Management: Effectively triage and manage an assigned caseload, identifying high-risk situations, and prioritizing interventions based on patient needs. Collaborate with colleagues to balance referrals and ongoing patient care responsibilities.
• Legal and Ethical Standards: Ensure compliance with legal and ethical regulations in all clinical assessments and interventions, including mandated reporting for suspected abuse or neglect and adherence to patient rights.
• Documentation: Maintain accurate and timely documentation, including interdisciplinary care plans, progress notes, and any pertinent paperwork, in alignment with organizational policies.
• Training and In-Service: Deliver educational sessions to hospital staff on psychosocial issues affecting patient care, contributing to a culture of learning and support.
• Quality Improvement Initiatives: Participate in the development and monitoring of quality improvement initiatives aimed at enhancing patient care outcomes within the hospice setting.
• Additional Duties: Engage in various departmental activities and educational programs designed for patients, families, and staff, contributing to a holistic approach to care.

This position primarily serves patients receiving hospice care, a demographic that often faces complex emotional, medical, and social challenges. The LCSW will work closely with terminally ill patients and their families, providing empathetic support and guidance as they navigate end-of-life decisions, grief, and loss. Understanding the unique needs and cultural sensitivities of this population is essential for effective service delivery.

• Master's degree in Social Work from an accredited institution.
• Minimum of 2 years of experience in a medical setting, with at least 1 year in oncology or a cancer-related environment.
• Licensed Clinical Social Worker (LCSW) certification in the state of California.
• Basic Life Support (BLS) certification from the American Heart Association is required.
• Fire Life Safety Training (LA City) certification must be obtained within 30 days of hire if not already certified.
• Excellent verbal and written communication skills, demonstrating the ability to effectively engage with patients, families, and professionals.
• Strong analytical, problem-solving, and planning capabilities.
• Proficient in functioning both independently and collaboratively within a team.
• Demonstrated commitment to patient-centered care and high standards of customer service.

While not mandatory, experience working specifically within a hospice or end-of-life care setting is highly beneficial. Familiarity with electronic medical records systems and integrated care pathways will be an advantage, enhancing workflow efficiencies and collaborative care practices.

The annual base salary for this position ranges from USD 120,000 to 130,000, reflecting the candidate’s experience, education, and specific job responsibilities.
