Job Description – Licensed Clinical Social Worker (LCSW)

Responsibilities:

  • Conduct biopsychosocial assessments.
  • Provide case management, utilization review, and care coordination.
  • Participate in care conferences and collaborate with multidisciplinary teams to ensure continuity of care.
  • Refer patients to appropriate disciplines and community resources.
  • Assess and intervene in high-risk situations, including suicide, homicide, self-harm, and patient safety concerns.
  • Collaborate with Child Protective Services when appropriate.
  • Provide support for drug/alcohol use disorders and treatment options.
  • Apply knowledge of cultural issues and adhere to the NASW Code of Ethics/Standards of Home Health Care.
