Position Summary

Lincoln County Health and Human Services is looking for a Family Nurse Practitioner to provide primary medical care services including assessment, diagnosis, and treatment to patients of the Health Center. This role is also responsible for p roviding holistic primary medical care for families and clients across the lifespan, emphasizing health promotion and disease prevention, and managing acute and chronic diseases in accordance with accepted medical standards and within the scope of practice.

Essential Functions/Major Responsibilities

Within the specialty of the Nurse Practitioner (NP), they are responsible for managing health problems encountered by the client and is accountable for health outcomes. The NP will follow the six standards of practice:

  • Assessment โ€“ Collect comprehensive data related to the health of the individual served. Synthesis and analysis of data and application of nursing principles and therapeutic modalities.

  • Diagnosis โ€“ Prescribing, dispensing, and administration of therapeutic devices and measures, including legend drugs and controlled substances as provided in Division 56 of the Oregon Nurse Practice Act, consistent with the definition of the family nurse practitionerโ€™s scope of practice.

  • Outcome identification โ€“ Based on the diagnosis the NP will identify outcomes that are culturally relevant, health literate, based in evidence and promotes the maintenance of health. Document expected outcomes and measurable objectives.

  • Planning โ€“ Develop a plan that is client-centered and reflects best practices by identifying strategies that are achievable.

  • Implementation โ€“ Oversee the implementation of the plan through treatment, case management, referrals to community partners, collaboration with care team, other health care providers and community resources, health promotion and education. Document activities and status of care coordination.

  • Evaluation โ€“ Apply on-going assessment and analysis of data, revise plan as needed, assures that interventions and activities are appropriate for ultimate health outcomes for the client. Document changes to enhance effectiveness of interventions.

Provide treatment of adult, adolescents, and pediatric clients in Primary Care settings:

  • Order lab work and other diagnostic tests as indicated as part of the diagnostic evaluation or ongoing management of the client. Follow-up on all abnormal findings, tests and/or lab values.

  • Consult and coordinate with physicians, Health Center Medical Director, and Primary Care Program Manager as indicated.

  • Assure the care team is functioning efficiently.

  • Document in Electronic Health Record to systematically collect, record and analyze data.

  • Provide preventive and maintenance care to patients and families consistent with accepted primary care practice and standard guidelines.

Provides leadership, guidance, and feedback to clinic staff to support a high-performance clinic that focuses on quality and safe patient care:

  • Aid in various program areas through educating and providing consultation to staff. Help keep staff informed of the programโ€™s overall activities and goals.

  • Serve as a consultant, accepting referrals from other health care providers in matters outside their expertise.

  • Communicate regularly with staff to review team relationships, specific problem areas and actions necessary for improvement.

  • Recognize team members' contributions and celebrates accomplishments.

Ensure quality assurance standards of care are met in programmatic areas:

  • Collaborate with Program Manager and Health Center Medical Director to:

  • Annually review and revise health care plans, standards of care and standing orders

  • Conduct periodic review of patient records

  • Participate in peer reviews to assure a high level of skills among other nurse practitioners

  • Participate in educational meetings as available and appropriate.

  • Audit records upon request from Program Manager or Health Center Medical Director to ensure appropriate data collection and documentation and evaluate care provided by other staff members upon request.

  • Ensure quality assurance standards are met through educating, monitoring and providing consultation to staff as appropriate.

  • Participate in the Quality Improvement Committee.

EDUCATION

Completion of a certified nurse practitioner school.

CERTIFICATIONS

Current Nurse Practitioner Certification as a family nurse practitioner in the State of Oregon at the time of appointment.

Possession of a National Certificate for Medicare, or the ability to obtain one by the end of trial service (6 full months from date of hire).

Driverโ€™s License

Oregon Driver's License required within 30 days of hire.

Other Licenses

Employee is responsible to maintain current and active Nurse Practitioner licensure/certification and to provide verification at time of renewal.

Desirable Qualifications: One or more yearsโ€™ experience as a nurse practitioner.

Knowledge of:

  • Maintain thorough knowledge of nursing principles, methods and procedures, and medical terminology.

  • Considerable knowledge of nurse practitioners clinical skills, preventive care and health maintenance associated with area of specialization.

  • Functions and resources of public and private agencies engaged in public or primary health care.

Skill or ability to:

  • Exercise considerable professional judgment as nurse practitioner in health assessment and delivery of health care.

  • Analyze health problems through physical exams, lab tests, counseling and medical histories, and to recommend health maintenance or treatment plans.

  • Train clients and general public on various preventive care and health maintenance techniques
