Key Responsibilities

• Provide direct nursing care to residents according to individualized care plans
• Administer medications and treatments as prescribed
• Monitor, assess, and document patient conditions and changes
• Assist with admissions, discharges, and resident assessments
• Collaborate with RNs, CNAs, physicians, and interdisciplinary team members
• Maintain accurate and timely clinical documentation
• Ensure compliance with infection control and safety protocols
• Deliver compassionate, patient-centered care

Requirements

• Active Licensed Practical Nurse (LPN) license required
• Previous long-term care or skilled nursing experience preferred
• Strong clinical assessment and communication skills
• Ability to work independently and within a healthcare team
• Must have reliable transportation
• Must comply with facility onboarding requirements
• Must bring state or federally issued identification and wear a photo badge at all times
• COVID-19 vaccination or approved exemption may be required per facility policy
