Job description

Job Summary:

The Licensed Vocational Nurse (LVN) provides participant-centered nursing care in both the PACE center and participants' homes under the direction of the Registered Nurse and/or Primary Care Provider. The LVN supports the interdisciplinary team by performing focused assessments (data collection), implementing the participant's plan of care, providing skilled nursing treatments, monitoring changes in condition, reinforcing participant education, coordinating services, and communicating findings to the RN and IDT. The LVN functions as an integral member of the interdisciplinary team, helping participants remain safely in the community while promoting quality outcomes and regulatory compliance.


Qualifications and Requirements:

Education: 

  • Current California LVN license in good standing 
  • Current BLS certification 
  • Valid California driver's license and reliable transportation 

Experience: 

  • Minimum of 1 year working with the frail or elderly or training by the PACE organization in care of the frail and elderly.

Skills and Knowledge: 

  • Excellent assessment skills
  • Excellent organizational skills 
  • Demonstrated ability to work in a multi-disciplinary team setting 
  • Ability to provide care in a clinic or home setting
  • Exhibits appropriate communication skills necessary for ages served (Adult/Geriatric)

Work Schedule:

Duties and Responsibilities:

Home-Based Nursing Services

  • Conduct scheduled nursing home visits as assigned by the RN or PCP. 
  • Perform focused nursing assessments and collect objective clinical data. 
  • Observe for changes in participant condition and promptly communicate findings to the RN and PCP. 
  • Reinforce participant and caregiver education established by the RN or PCP. 
  • Assess medication compliance and identify barriers. 
  • Perform medication reconciliation during home visits. 
  • Evaluate home safety concerns and report environmental risks. 
  • Monitor skin integrity and perform wound care according to physician orders and organizational protocols. 
  • Perform catheter care, ostomy care, and other approved skilled nursing procedures. 
  • Obtain laboratory specimens and point-of-care testing as appropriate. 
  • Assist with chronic disease monitoring including: 
    • Diabetes 
    • CHF 
    • COPD 
    • Hypertension 
    • Weight monitoring 
  • Monitor participants following hospitalization or SNF discharge. 
  • Document findings in the electronic medical record. 

Clinic Nursing Responsibilities

  • Prepare participants for clinic appointments. 
  • Obtain and document vital signs. 
  • Perform medication administration. 
  • Administer injections and immunizations as permitted by California law and organizational policy. 
  • Assist providers during examinations and procedures. 
  • Perform dressing changes and wound treatments. 
  • Perform venipuncture and specimen collection. 
  • Initiate IV therapy only if appropriately certified and permitted under standardized procedures. 
  • Assist with urgent participant needs. 
  • Monitor participants following treatments. 
  • Coordinate follow-up appointments.

Care Coordination

  • Communicate significant changes in participant condition immediately to the RN. 
  • Participate in daily morning meetings. 
  • Attend IDT meetings as requested. 
  • Provide updates regarding participant status. 
  • Assist with implementation of IDT interventions. 
  • Coordinate services with home care, transportation, rehabilitation, social work, and clinic staff. 
  • Support transitions of care following hospitalizations. 

Participant & Caregiver Education

  • Reinforce established teaching plans. 
  • Educate participants regarding: 
    • Medication adherence 
    • Fall prevention 
    • Disease management 
    • Infection prevention 
    • Skin care 
    • Nutrition recommendations developed by the IDT 
  • Verify participant understanding through teach-back. 

Documentation

  • Complete documentation within organizational timeframes. 
  • Document nursing interventions accurately. 
  • Report incidents according to policy. 
  • Maintain confidentiality in accordance with HIPAA. 

Quality & Regulatory Responsibilities

  • Participate in quality improvement activities. 
  • Assist with audit preparation. 
  • Follow CMS, DHCS, OSHA, and infection prevention requirements. 
  • Maintain required competencies.
