Job description

Salary: $55,000 - $72,000/year

Under the direction of the CEO, the PACE Outreach Coordinator is responsible for coordinating K-day PACEs marketing, enrollment, and community engagement functions. This includes developing and maintaining relationships with referral sources such as community groups, coordinating and delivering program presentations, and building partnerships with local Medi-Cal departments and level of care assessors to streamline the enrollment process. The Outreach Coordinator also assists in developing and executing annual marketing plans, written materials, and advertising campaigns in collaboration with leadership. They maintain referral tracking systems and marketing files, and oversee participant enrollment to ensure K-day PACE meets or exceeds census goals.


Qualifications and Requirements:

  • Education: Bachelors Degree required. Masters Degree preferred.
  • Experience: Minimum of two (2) years of experience in a marketing or sales role (health care provider preferred). Experience in the field of health or human services is preferred but not required.


Duties and Responsibilities:

  • Coordinate and execute outreach, PR, and community engagement activities to ensure consistent achievement of monthly referral, home visit, and enrollment goals.
  • Meets or exceeds census enrollment targets.
  • Provides individual and group marketing presentations to potential referral sources.
  • Identify opportunities to collaborate with community organizations and execute against those opportunities in a thoughtful manner that reflects our mission and value proposition.
  • Establish and maintain best-in-class relationships with community leaders and partners, including physicians, hospital discharge planners, post-acute facilities, and other potential referral sources in the service area.
  • Develop and implement an annual marketing plan to achieve monthly enrollment goals with the Marketing Director, following State and Federal guidelines.
  • Provide education to referral sources in the community regarding K-day PACE services.
  • Demonstrate in-depth knowledge of all relevant components of the Marketing Playbook.
  • Comply with all rules on allowable and prohibited marketing practices.
  • Identify multiple potential opportunities for process improvement.
  • Must be willing and have the ability to work a varied schedule that may include evenings, weekends, and overtime.
  • Complete all required documentation and generate reports in a timely and accurate manner.
  • Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
  • Follow K-day PACE policies and procedures and participate in any required
  • Quality Improvement activities, staff training, and meetings.
  • Communicate regularly with the Supervisor and team regarding workload and priorities.
  • Timely completion of all mandated training and education.
  • Timely completion of all mandated occupational health screenings as needed.
  • Exercises flexibility in performing assignments as business needs evolve.
  • Acts as Enrollment Specialist when necessary.
  • Evaluates and analyzes data and activities associated with marketing and the enrollment process weekly, monthly, and quarterly.
  • Evaluates and re-evaluates processes to maximize efficiencies.
  • Collects data and prepares reports in accordance with the State PACE Administering agency and CMS requirements.
  • Other duties as assigned.
