Job description

Mountain View Hospital is looking for a Certified Medical Coder to join our team!

JOB SUMMARY:

Accountable for conversion of diagnoses and treatment procedures into codes using an international classification of diseases.  The coder assigns ICD-10-CM, ICD-10-PCS, CPT, and/or HCPCS codes creating APC or DRG group assignment for reimbursement purposes.  Requires skill in the sequencing of diagnoses/ procedures to optimize reimbursement.  Must be able to read and interpret operative reports, history and physicals, physician orders, and pathology reports to determine the correct coding.  Ensures that records are coded in an accurate and timely manner.  Abstracting worksheets to add codes in software.

SPECIFIC DUTIES AND RESPONSIBILITIES:
 

  1. Ensure that records are coded.
  2. Reviews charts thoroughly to ascertain all diagnoses/procedures.
  3. Contacts responsible physician in professional manner if diagnosis is not available on chart.
  4. Coder’s diagnoses and procedure on clinical summary agree with physician’s preference 100% of the time.
  5. Refers chart to director, if there is a question regarding the diagnosis code.
  6. Utilizes computerized coding/abstracting equipment.
  7. Codes all diagnoses/procedures in accordance to ICD-10 coding guidelines.
  8. Logs diagnoses, procedures and other abstracting data on worksheet to input in software.
  9. Ensures data quality and optimum reimbursement allowable under the federal and state payment system.
  10. Acts as a resource person to hospital staff for coding and may provide education regarding coding changes/issues.
  11. Must be familiar with all charting and coding requirements.
  12. Demonstrates an ability to be flexible, organized and function well in stressful situations.
  13. Treats patients and their families with respect and dignity; ensures confidentiality of patients’ records.
  14. Maintains a good working relationship both within the department and with other departments.
  15. Participates in educational program and in-service meetings. 
  16. Attends meetings as required.
  17. Accepts and completes other duties as assigned or requested.
  18. Maintains patient confidentiality at all times.
  19. Manages and operates equipment safely and correctly.
  20. Maintains 92% coding accuracy.
  21. Completes annual education requirements
  22. Maintains regulatory requirements.
  23. Reports to work on time and as scheduled, completes work within designated time.
  24. Wears identification while on duty, uses computerized punch time system correctly.
  25. Adheres to dress code, appearance is neat and clean.
  26. The employee is required to comply with all PCI standards and other company policies and procedures for payment card acceptance and security.
  27. Establish and comprehend Risk Coding; responsible for ensuring Risk codes are appropriate and to the highest complexity

Education/Certification:   High School diploma or general education degree (GED).  Knowledge of diagnoses/procedures in accordance with ICD-10 and CPT coding guidelines for acute care facilities. Must be RHIA, RHIT, CCS, CCA or CPC certified.

Experience:  One year or more related experience and/or training; or equivalent combination of education and experience preferred.

Equipment/Technology:  Ability to use abstracting and coding software, computer, fax machine.

Language/Communication:  Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.  Ability to write routine reports and correspondence.  The ability to speak effectively before groups of customers or employees of organization.

Mathematical Skills:  Basic math skills

Mental Capabilities:   Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.  Ability to deal with problems involving several concrete variables in standardized situations.

Interpersonal:  Ability to work well with others, routine reports and correspondence. Fully shares information and expresses ideas, opinions and reaction constructively.  Seeks feedback and gives others constructive feedback.
