In this role, you'll provide exceptional physical therapist (pt) services in the comfort of patients' homes, focusing on functional improvement, independence, and quality of life enhancement. Key Responsibilities

•        Patient Assessment: Conduct comprehensive evaluations of patients' physical conditions, mobility, strength, balance, and functional abilities in their home environment

•        Treatment Planning: Develop individualized treatment plans with measurable goals focused on improving mobility, reducing pain, and restoring functional independence

•        Therapeutic Interventions: Implement evidence-based physical therapy techniques including therapeutic exercises, manual therapy, gait training, and balance activities

•        Patient & Caregiver Education: Provide expert guidance on home exercise programs, fall prevention strategies, proper use of assistive devices, and safe mobility techniques

•        Documentation Excellence: Utilize WellSky Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments

•        Care Coordination: Collaborate effectively with physicians, nurses, other therapists, and healthcare team members to ensure comprehensive patient care

•        Progress Monitoring: Regularly reassess patient progress, adjust treatment plans as needed, and ensure adherence to clinical standards and regulatory requirements

•        Safety Assessments: Evaluate home environments for fall risks and recommend modifications to enhance patient safety

Required QualificationsLicensure & Certifications

•        Graduate of an accredited Physical Therapy program (DPT or equivalent)

•        Valid Illinois Physical Therapist license

•        Current CPR certification

•        Valid driver's license and auto insurance

Experience & Skills

•        Open to new graduates and experienced physical therapists - comprehensive training provided
