Home Health LVN Responsibilities:

  • Provide one-on-one nursing services in patientsโ€™ home.
  • Provide education, wound care, draw labs, Foley care, ostomy care and more.
  • Expect 3-7 visits per day serving a population of elderly and adult patients (depending on status)
  • Demonstrate efficient teamwork with the interdisciplinary team and office staff.
  • Monitor reactions and patient progress using observation, assessment, and evaluation skills.
  • Educate patients and family members according to disease process, medications, POC, treatment options, and home care procedures according to the plan of care.
  • Report to physician and RN of adverse findings.
  • Comply with HIPAA regulations in and out of the office.
  • Follow infection control policy in and out of the office.
  • Document visits in EMR system (Wellsky) in a timely manner.
  • Must be IV Certified

Coverage Areas: Full-Time Positions

  • Roseville, Rocklin, Lincoln, and Antelope (Full-Time)

Schedule for this Position Home Health LVN:

  • Monday โ€“ Friday (flexible hours), rotating on call once every 4th week.
