CV RN Program Coordinator - Structural Heart Program

The RN Specialty Program Coordinator for Structural Heart supports patients across the continuum of structural heart care through education, coordination, and interdisciplinary collaboration. This role works closely with physicians, advanced practice providers, nursing, and operational teams to manage referrals, patient intake, clinic support, and procedural coordination for advanced structural heart interventions, including transcatheter aortic valve replacement (TAVR) and MitraClip procedures. The coordinator partners with interdisciplinary teams to ensure program quality measures and performance metrics are met, making this a strong fit for an RN who values patient connection, teamwork, and contributing to high-quality specialty care without providing direct bedside care.
JOB SUMMARY
Works under limited supervision to Coordinate efforts with Health System Physician Referring Service, Health System liaisons, RN coordinators and clinic management to provide stream-lined, efficient systems of referral management and service recovery.

EXPERIENCE AND EDUCATION
Required
  • Education
    Bachelor's Degree - BSN
  • Experience
    3 years of nursing experience
  • Licenses and Certifications
    (RN) REGISTERED NURSE Upon Hire and
    (BLS) BASIC LIFE SUPPORT course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire

JOB DUTIES
  • Receives and provides immediate responses to inquiries and patient referrals. Establishes immediate relationship between referral sources and hospital.
  • Serve as the clinical link between the referring physician office and HLV. Facilitates communication with referring doctors for patients seen in clinics and for patients discharged from the hospital.
  • Coordinates efforts with Health System Physician Referring Service, Health System liaisons, RN coordinators and clinic management to provide stream-lined, efficient systems of referral management and service recovery.
  • Coordinates bed placement and medical transportation as needed
  • Interfaces with other hospital departments (I.e., infection control, admitting, pastoral care, and nursing) and physicians to facilitate admission process.
  • Interfaces with patients as a liaison to facilitate their first visit at UTSW and assists in obtaining records before new patient evaluation.
  • Assists with obtaining clearances needed for treatment and care
  • Works closely with interdisciplinary core team and oversees referral/intake process to ensure patient has all necessary items needed for meaningful visit
  • Assign clinical duties as needed to ensure effective workflows, patient care requirements are meet and clinical quality and safety measures are followed.
  • Resource to patients, and families, regarding social services, financial aid and outside resources available
  • Reviews referrals, verify information. Determines appropriateness of referral based on established criteria and assures that all patient work up and resulting reports are provided in accordance with protocols.
  • Establishes & maintains infrastructure to monitor referring physician practice patterns; alerts team to any change in referral patterns.
  • Tracks data regarding intake and referral sources.
  • Coordinates efforts with Health System Physician Referring Service, Health System liaisons, RN coordinators and clinic management to provide streamlined, efficient systems of referral management and service recovery.
  • Responsible for training future/new coordinators
  • Development and maintenance of process maps detailing throughput from intake to procedure
  • Development and maintenance of education materials for staff and patients.
  • Participates in clinic, facilitates work up and provides education as appropriate
  • Participates in inpatient rounding and/or post procedure rounding as deemed appropriate by team
  • Works closely with Quality team to include but not limited to: maintaining and validating data, investigating fall outs, performing RCA, participating and/or leading PI projects
  • Ensures all quality metrics are completed by staff during clinic visit(s) per protocols by staff the meet registry requirements
  • Provides on-call coverage as deemed appropriate by team.
  • Performs other duties as assigned.
