We are seeking a Full-Time Home Health RN Clinical Manager. Below is a brief list of what we are seeking. Salary is commensurate with experience ($95K - $100K).
- Five (5) years of Home Health experience (Medicare knowledge a must)
- At least three (3) to five (5) years in a RN clinical management role
- Working knowledge of SHP
- Must be clinically sound and able provide training to new clinicians in the community.
- Understands VBP and PDGM impact on home health services
- Extensive OASIS knowledge
- An understanding of Intake and Scheduling processes
- Able to manage supplies during episodic care
- Strong knowledge of wound care a plus
- Willing to assist with Scheduling, Clinical Issues, and Patient Visits
- Willing and able work ON-CALL at least once per month
System Knowledge – Must be system savvy (able to toggle between multiple screens and systems simultaneously)
Matrix Care (Preferred)
MS Office (Excel, MS Teams, Outlook, etc.)

- Be ready to JUMP IN and assist with challenges as they arise for our clinicians, Intake, and Scheduling staff

- Have a Crisis Management mindset
- Must reside and be licensed in the State of Illinois
