The Registered Nurse Assessment and Referral Counselor provides telephonic and in-person risk assessment and crisis intervention, initial contact with referral sources and potential patients, coordinates the referral and intake process, determines appropriate level of care, facilitates the admission process and maintains communication with referral sources, families, patients and unit staff.

ESSENTIAL JOB FUNCTIONS:
  • Perform initial medical screening
  • Perform admission assessments
  • Assess, direct and intervene in crisis situations
  • Medical Screenings - Perform nurse to nurse screening by phone on patients being transferred from other facilities
  • Obtain/document pre-certification on admissions as necessary
  • Basic Medical Triage: skin check, wounds, etc.

    Qualifications
    Education: Associates or Bachelor's Degree in Nursing
    Experience: Prior intake, assessment, admission and crisis intervention experience with a psychiatric mental health population, preferable in an inpatient setting.
    Licensure/Certification: Currently licensed to practice by the Louisiana State Board of Nursing as a Registered Nurse.
    Knowledge: Prefer knowledge of crisis intervention and risk assessment, psychiatric diagnoses symptomatology, basic chemical dependency, age specific growth and development and crisis behavior management.
