Job description

Location: Leominster, MA - Outpatient Treatment Program
Schedule: Requiring early mornings at least 3 days per week. Available days are Monday- Friday or Tuesday-Saturday (once fully trained)
Pay rate: $22.90-$26.51
The Recovery Support Navigator is responsible for:
  • Providing quality services and case management to assigned patients in accordance with the Recovery Navigator model.
  • Performing intake assessments on all new patients to the program and completes all associated documentation within required time frames.
  • Working with the patient to develop and implementing an individualized plan with goals and objectives based on identified needs.
  • Assisting patients with scheduling and coordinating medical and behavioral health appointments.
  • Coordinating care and the plan with existing providers including but not limited to behavioral health, primary care and Community Partner.

Qualifications:
  • Bachelor's Degree in human services related field or discipline
  • Or LADC
  • Or has a recognized certification and/or licensure for the target population to work with
  • Or sufficient experience and training in counseling and meets regulatory requirements
