HOW YOU'LL MAKE AN IMPACT

The Care Navigator: Intake Specialist position is an administrative job with responsibility for the intake of all new clients.This position will be responsible for administering mental health screenings, physical health measurements, and recording required client profile fields. The Duties may include but are not limited to:

  • Provide exceptional customer service;
  • Process referrals from internal and external parties
  • Prepare and open charts for new clients;
  • Schedule new client appointments and assign appropriate clinician;
  • Obtain and verify client insurance
  • Meet with new clients to review and sign client forms and information
  • Take client's vitals
  • Perform data entry
  • Travel to other CMMHC locations for coverage
