As a Home Health Intake Coordinator, you'll serve as a vital connection between patients, families, referral sources, and clinical teams, ensuring referrals are processed accurately, admissions are coordinated efficiently, and patients receive the care they need without unnecessary delays. Your attention to detail and commitment to exceptional service will help create a positive first impression while supporting continuity of care.

This role is ideal for someone who is highly organized, customer-focused, and enjoys coordinating multiple priorities in a fast-paced healthcare environment. You'll manage patient intake and data entry, verify referral information, audit admission documentation, coordinate scheduling, communicate with patients and community partners, and monitor documentation to ensure timely admissions and regulatory compliance. Through strong communication, accuracy, and proactive problem-solving, you'll play an essential role in supporting high-quality patient care and the continued success of the home health team.

Locationย 

  • Office Location: Bingham Farms, MI

Hoursย 

  • Monday to Friday, 11 a.m. to 7:30 p.m., with weekend rotation

Required Qualificationsย 

  • High School Diploma or GED
  • Minimum 1 year experience in health care data entry
  • Reliable transportation, valid driver's license, and valid automobile insurance coverage

Preferred Qualificationsย 

  • Associates Degree
  • Experience in home care or hospice information system
