Requirements/Qualifications

  • A regulated health professional (RPN) with a minimum 2 years experience
  • Must be willing to travel in the Mississauga and Halton region
  • Current BLS certification
  • Police Criminal Record Check with Vulnerable Sector Screening
  • 2 Steps of Tuberculosis (TB) Test and Immunizations (up to date) 
  • Excellent communicator, able to foster relationships with Home Care Staff and clients
  • Strong time management skills, confidentiality, and professionalism

Responsibilities:

  • Conduct initial and ongoing in-home assessments of health status, mobility, cognition, and home safety
  • Provide clinical care including medication administration, injections, IV therapy, wound and catheter care, and ostomy management
  • Perform diagnostic procedures (e.g., blood glucose and urine tests) and manage medical equipment such as oxygen, feeding tubes, or CPAP
  • Complete intake interviews and welcome calls using standardized script
  • Review referrals, medical records, and insurance eligibility 
  • Educate patients and caregivers on medication use, wound care, symptom monitoring, and equipment management.
  • Coordinate care and communicate with physicians, pharmacies, and other healthcare providers to ensure continuity of care
  • Document all assessments, treatments, and observations accurately in the electronic health record
  • Follow infection control, safety, and privacy standards (PIPEDA).
  • Support discharge and transition planning by arranging community resources and patient follow-up
  • Participate in quality improvement initiatives and audits
