Job Title
Licensed Master Social Worker (LMSW) โ€“ Part-Time / PRN
Job Summary
We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our Home Health and Hospice team on a Part-Time or PRN basis. The LMSW will provide psychosocial assessments, counseling, emotional support, resource coordination, and advocacy services to patients and families experiencing illness, disability, end-of-life care, and bereavement. The LMSW will work collaboratively with physicians, nurses, therapists, and other interdisciplinary team members to ensure high-quality patient-centered care.
Essential Duties and Responsibilities
  • ย Conduct comprehensive psychosocial assessments of patients and families.ย 
  • ย Evaluate social, emotional, financial, environmental, and community support needs.ย 
  • ย Develop and implement individualized care plans in collaboration with the interdisciplinary team.ย 
  • ย Provide counseling, crisis intervention, and emotional support to patients and caregivers.ย 
  • ย Assist patients and families in accessing community resources, financial assistance programs, and support services.ย 
  • ย Educate patients and families regarding available healthcare, hospice, and community services.ย 
  • ย Facilitate advance care planning and discussions regarding patient goals and preferences.ย 
  • ย Participate in interdisciplinary team meetings and contribute to patient care planning.ย 
  • ย Maintain accurate, timely, and compliant clinical documentation.ย 
  • ย Coordinate discharge planning and transitions of care when appropriate.ย 
  • ย Provide bereavement support and grief counseling to families and caregivers.ย 
  • ย Ensure compliance with federal, state, agency, Medicare, and hospice regulations.ย 
  • ย Maintain patient confidentiality in accordance with HIPAA requirements.ย 
  • ย Perform other duties as assigned by the Clinical Director or Administrator.ย 
Qualifications
Education
  • ย Master's Degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE).ย 
Licensure
  • ย Current Licensed Master Social Worker (LMSW) license in the State of Texas.ย 
  • ย Current driver's license and reliable transportation.ย 
  • ย Current automobile liability insurance.ย 
Experience
  • ย Minimum one (1) year of social work experience preferred.ย 
  • ย Experience in Home Health, Hospice, Healthcare, Case Management, Geriatrics, Palliative Care, or Medical Social Work preferred.ย 
  • ย Knowledge of Medicare, Medicaid, and community resources preferred.ย 
Skills and Abilities
  • ย Strong assessment, counseling, and crisis intervention skills.ย 
  • ย Excellent verbal and written communication abilities.ย 
  • ย Ability to work independently and as part of an interdisciplinary team.ย 
  • ย Strong organizational and time-management skills.ย

Physical Requirements

  • ย Ability to utilize a computer, tablet, and other office equipment.ย 
Work Schedule
  • ย Part-Time and PRN positions available.ย
