Job Description:

The Care Team Operations Associate plays a key role in supporting members who we see clinically in social and care management needs. This position serves as support in administrative tasks and follow up for connecting members to their needs, by direction of the assigned Community Health Partners and Nurse Care Managers to the member.

Responsibilities
  • Coordinates home health, insurance, vendors, and other social assistance services for our members

  • Initiates and manages referrals based on member needs, ensuring effective communication throughout the process.

  • Connects with insurance, other providers, and other organizations to request and deliver appropriate medical services and records for members

  • Uses our analytical platforms to generate and communicate weekly reporting & planning to support clinical care

  • Uses member benefits platforms to determine eligibility as needed, organize Medicare/Medicare Advantage support and enrollment as needed

  • Communicates with members and external agencies regarding referrals made on their behalf.

  • Supports team meetings with note-taking and follow-ups on all commitments

Work Experience
  • 1 year administrative work such as scheduling and coordination tasks (appointments, follow-ups, or care plans) and or experience in maintaining accurate records and documentation.

  • Must be BILINGUAL - SPANISH/ENGLISH
