Key Responsibilities
  • Collaborate with primary care providers to identify and manage mental health issues, contributing effectively within the interdisciplinary primary care team.

  • Provide brief, targeted interventions for patients requiring mental health support, ensuring that care is aligned with the established Agency and Federally Qualified Health Center (FQHC) guidelines.
  • Monitor patient progress through regular follow-ups and modify treatment strategies as necessary, ensuring optimal mental health outcomes.
  • Develop relapse prevention plans and assist patients in maintaining stable functional levels in their daily lives.
  • Identify at-risk patients and implement preventive strategies to mitigate the worsening of their conditions.
  • Coordinate and monitor the delivery of behavioral healthcare services, collaborating with other providers as necessary, with patient consent.
  • Accurately document patient progress and diagnostic considerations in electronic health records, keeping primary care providers informed about treatment needs and progress.
  • Offer consultation to management and team members regarding behavioral health programs and potential outcome evaluations.
  • Support patients in adhering to medical treatments prescribed by primary care providers, including discussing strategies for managing any side effects of medications.
  • Uphold the Code of Ethical Conduct for Clinical Professionals while carrying out other related duties as assigned.

Compensation and Benefits
