Fashion Valley Comprehensive Treatment Center (CTC), located in San Diego, CA, is part of Acadia Healthcare's Comprehensive Treatment Centers, the leading provider of medication-assisted treatment in the nation. Our clinical team provides compassionate, high-quality counseling and therapy to patients that are seeking recovery from Opioid Use Disorder.

Your Job as a Case Manager:

The Case Manager is a clinical professional who manages the patients' cases with a solid background of clinical knowledge and expertise and may be responsible for providing some therapeutic services to patients. The Case Manager provides quality care through developing, implementing, managing and evaluating patient care plans. 

Your Responsibilities as a Case Manager:

  • Provides quality care through developing, implementing, managing, and evaluating patient care plans;
  • Collects clinical data, correlates this clinical data, and provides clinical input to the treatment team;
  • Conveys medical criteria and clinical information between the insurance provider and treatment team as warranted;
  • May also correlate clinical data for the business office as required.
  • Providing case management and care coordination services for patients in the clinic;
  • Responsible for coordinating the continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements;
  • May also be responsible for conducting individual, and family educational sessions, various duties related to coordinating treatment and discharge activities, as needed;
  • Coordinates in a timely manner, issues or activities relevant to the team, and/or to the team's relationship within the facility or treatment milieu; and,
  • Perform other duties as assigned.

Compensation for roles at Fashion Valley CTC varies depending on a wide array of factors including but not limited to the specific location, role, skill set, and level of experience.As required by local law, Fashion Valley CTC provides a reasonable hourly range of compensation for roles that may be hired in California as set forth below.Role Location: California

Calculated Salary Range for role: $29.14 - $38.00 per hour.

Qualifications

Your Skills and Qualifications as a Case Manager:

  • Bachelor's degree in Social Work, Counseling or related Human Service field.
  • 1-2 years of experience working in behavioral health setting. Experience with the population served by the clinic preferred.
  • Previous experience in case management and/or utilization review preferred.

Licenses/Certifications: 

  • AOD Registration is required.
  • AOD Certification is preferred.  
  • Registration/Certification must be obtained from a California-approved Counselor Certification Organization (CAADE, CADTP or CCAPP).
