About the Role
As a Case Coordinator, you will provide individualized skills training and support to youth, families, and adults in a variety of settingsโ€”including homes, schools, and the community. Youโ€™ll play a key role in helping clients build daily living skills, improve social functioning, and increase independence, all while fostering meaningful relationships and measurable progress.

What Youโ€™ll Do

  • Develop and implement personalized training plans based on each clientโ€™s unique goals and needs
  • Provide engaging one-on-one and group sessions focused on daily living skills, social skills, and parenting education
  • Apply evidence-based practices and creative approaches to support learning and growth
  • Offer ongoing mentorship and encouragement to clients in a supportive, positive environment
  • Help clients apply new skills in real-world situations to build confidence and independence
  • Collaborate closely with families, caregivers, and service providers to ensure holistic care
  • Participate in team meetings and case conferences to monitor progress and adjust plans
  • Maintain accurate documentation, progress notes, and reports
  • Connect clients with community resources and support their participation in community activities
  • Promote social engagement and community integration
  • Stay current with best practices through ongoing training and professional development
