Job Title: Occupational Therapist (Home Health)

Location: Iowa City, Iowa
Job Function: Non-Managerial Clinical

Job Description

At Innovive Health, our Occupational Therapists provide one-on-one care with the autonomy to manage their schedules and focus on meaningful, functional outcomes. You will work with medically stable patients in the community, helping them improve independence and quality of life from where they reside. Looking for flexibility and more control over your schedule? Join Innovive Health as a PRN Occupational Therapist, where you can pick up visits based on your availability

What You’ll Do:
  • Conduct home occupational therapy visits including evaluations, follow-up visits, 30-day re-evaluations, recertifications, and discharges
  • Evaluate patient rehabilitative status and contribute to the development of the plan of care
  • Initiate occupational therapy programs and guide other personnel involved in patient care
  • Communicate with physicians regarding therapy frequency, duration, and goals
  • Prepare and submit clinical documentation and progress summaries in a timely manner
  • Educate patients and caregivers on treatment plans and home exercise programs
  • Assist patients in achieving functional goals through therapeutic exercises, transfer training, neuromuscular re-education, and coordination activities
  • Provide education on safety and fall prevention to improve participation in activities of daily living
  • Assess vital signs including blood pressure, heart rate, and oxygen saturation as needed and report abnormalities
  • Educate patients on the proper use of durable medical equipment (DME) and recommend equipment as appropriate
  • Document patient progress and adjust treatment plans based on clinical findings
  • Coordinate discharge planning with physicians, nurses, social workers, and other care team members
  • Collaborate effectively with interdisciplinary team members including SN, OT/COTA, PT/PTA, SLP, and physicians
  • Maintain patient confidentiality and comply with all organizational policies and procedures
  • Follow infection control and safety protocols at all times
  • Maintain professional knowledge through continuing education and professional development
  • Perform other duties as assigned
What You Have:
  • Graduate degree in Occupational Therapy from an accredited university
  • Valid Occupational Therapy license in the state of Iowa
  • Current CPR certification
  • Minimum 1 year of experience in home care, skilled nursing facility, acute care, or inpatient rehab
  • Valid driver’s license
  • Access to a reliable vehicle and active car insurance
Preferred Education and Experience:
  • 1 year of home health experience preferred
Physical Demands and Work Environment:
  • Frequently required to stand, walk, and sit
  • Continual use of hands and fingers
  • Frequent bending, stooping, kneeling, and climbing
  • Occasional lifting or pushing of weights up to and over 25 pounds
  • Occasional patient lifting or transfers
  • Exposure to bloodborne and airborne pathogens or infectious materials
Travel Requirements:

Daily local travel required to perform patient visits and attend meetings or training. Occasional travel to corporate offices outside the assigned region may be required.
