POSITION SUMMARY: Reporting to the State Director, the Incident Management Coordinator (IMC) ensures reportable event requirements are met with all state and payor entities. This role ensures that client services are delivered in a safe, efficient, and effective manner, in accordance with established quality standards, and applicable state and federal regulations. This role acts as the Incident Management Coordinator as required by regulation and contracts.
Hours: Full Time: Monday through Friday 8 am to 5 pm.

ESSENTIAL DUTIES:
  • Files and records all reportable incidents/events to appropriate entities (Licensure, REM, APS, Care Coordinator, etc.) within the required timeframe.
  • Schedule and facilitate incident/event discussions with Operations, Quality, HR and/or Compliance as needed.
  • Lead Tier 2 Investigations with the support of the branch staff
  • Coordinate investigations with Operations, HR, Quality and/or compliance (includes employee fraud and employee misconduct events).
  • Coordinate with the branch to ensure incident/event follow-up is completed, i.e., employee training is completed, more information as needed.
  • Monitor the incident/event outcomes by reaching out to entities, such as APS and MCO Care Coordinators as needed until the final resolution is made, i.e., substantiated, or unsubstantiated.
  • Monitor reportable events and client service indicators and metrics and implement required corrective actions to resolve problems and ensure that all service quality and efficiency objectives are met.
  • Provides incident report submissions and follow up to the QA Manager & State Director
  • Participate in regional home care industry organizations to follow incident management and reportable event trends and developments and identify potential training and program enhancement opportunities.
  • Manage Relias Training for all field and admin staff (includes initial and annual reportable event training)
  • Conduct periodic audits of client and employee files to ensure compliance with contract specifications, Company policies and state and federal regulations.
  • Maintain a high degree of confidentiality at all times due to access to sensitive information.
  • Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the department.
  • Follow all Medicare, Medicaid, and HIPAA regulations and requirements.
  • Abide by all regulations, policies, procedures, and standards.
  • Perform other duties as assigned.
Position Requirements & Competencies:
  • Excellent oral and written communication skills
  • Ability to multi-task under tight deadlines
  • Strong communication skills and interpersonal skills
  • A valid driver's license and proof of insurance is required.
  • Must be a certified TN Provider Investigator
  • Experience completing critical incident reports
  • Experience working as part of a TN Provider Reportable Event Review Team (PRERT).
  • Bachelor's degree in business, Social Services, Public Administration, or a related field of study is preferred
  • Experience working within a managed healthcare environment as an employee or contracted provider is preferred.
