Nurse Practitioner - Hybrid Primary Care (Home Visits + Clinic) | Williamsburg, NY
Outpatient practice in Brooklyn is seeking a full-time Nurse Practitioner to provide a mix of home-based and office-based primary care in Bedford-Stuyvesant. This role is ideal for a provider who enjoys relationship-driven medicine and delivering care directly in the community.
Position Overview
The Nurse Practitioner will provide comprehensive primary care across two care settings: in-office visits and scheduled home visits. The schedule is structured around approximately 2-3 days of home visits and 2-3 days in the clinic.
You'll manage chronic conditions, conduct wellness visits, perform routine follow-ups, and support preventative care in a fully outpatient environment. The practice offers strong administrative support and a steady patient base.
Qualifications
  • Nurse Practitioner (FNP, AGNP, or similar)
  • Active New York NP license
  • Outpatient primary care experience preferred
  • Comfortable with hybrid care model (home visits + clinic)
  • Strong communication and autonomy
